Insights from COVID -19 Pandemic Management : Strengthening of Health Systems in Pakistan – A Qualitative Study

Abstract

COVID-19 pandemic witnessed an unprecedented global health crisis, profoundly affecting countries economically and socially. Despite being extremely vulnerable to the pandemic, Pakistan responded better as compared to other developing and developed countries. World Health Organization (WHO) acknowledged Pakistan among seven countries from which the international community can learn to handle future pandemics. This study finds out the main challenges faced by public health institutions in the management of the COVID-19 pandemic and explores the strategies used to manage these challenges. It also identifies key takeaways from public health institutions’ experience of managing the pandemic to strengthen health systems in Pakistan and determines the role of governance, leadership and digitalization in Pakistan’s response. Six in-depth, semi-structured interviews were conducted with key stakeholders, and representatives from public health institutions who played pivotal roles in the national COVID-19 response as methods for data collection. To facilitate the interviews, comprehensive interview guides were developed specific to each participant and their institution. The interviews were taken in-person, recorded, and supplemented with notetaking. Furthermore, the study incorporated a thorough review of case studies and pertinent documents to provide additional context and depth to the qualitative findings. A survey using a structured questionnaire was used to gather demographics and basic information from the participants. Thematic analysis was conducted. Transcripts were coded using thematic analysis and codes were grouped into broader themes reflecting the main topics discussed by participants. Key findings, conclusions and recommendations were framed to provide key insights derived from the management of the COVID-19 pandemic in Pakistan to guide the enhancement of Pakistan’s health systems and future pandemic preparedness. Six themes were identified 1) Pre-Pandemic Preparedness and capacity building; 2) Challenges faced at the different stages of managing the pandemic; 3) Coping strategies for managing challenges at various stages of the pandemic; 4) Role of governance and leadership in Pakistan’s pandemic management; 5) Role of digitalization in Pakistan’s pandemic management and 6) Strengthening Pakistan’s health systems: key takeaways. The multifaceted strategies adopted by Pakistan during the pandemic illustrate the importance of adaptability, resourcefulness, and coordinated efforts in managing public health crises of such magnitude. From establishing quarantine centers and expanding diagnostic capacities to implementing "smart" lockdowns and managing vaccination challenges, each initiative contributed to an effective pandemic response. These experiences highlight valuable lessons for future public health emergencies, emphasizing the need for comprehensive planning, collaboration across levels of government, and innovation in public health crisis management. The study offers recommendations on how Pakistan can strengthen its health systems to enhance preparedness for future pandemics and use insights to fight persistent health issues, such as polio, diabetes and malnutrition crises.
